Provides health education outreach program activities for large participant groups in identified counties throughout Tennessee and surrounding areas to help maximize and maintain healthy lifestyles. Responsible for the management of challenging group dynamics in order to assure proper program delivery. Serves as a resource to assist individuals, other healthcare workers and the community. Models appropriate behavior as exemplified in MLH Mission, Vision and Values. Presents health information to patients, families, or community groups in order to promote healthy lifestyle decisions and chronic disease management. Provides health education and awareness to the assigned community program. Responsible for large group participant management, especially groups with behavioral issues and/or special needs. Enters data, documents and evaluates the effectiveness of program activities. Requires Bachelor's Degree in Health Services, Education, or Social Services. Requires 1-3 years of customer service, collections or call center experience. Proficiency in Microsoft Office and ability to operate standard office equipment. Must be able to assess needs of participants and their families and respond appropriately. Strong communication, organizational, coordination, and time management skills required. Physical demands include light work with occasional exertion up to 25 lbs, frequent travel, and exposure to patient bodily fluids and hazardous materials.
